Have all insurance policies available that cover each auto owned by anyone living at the address of the person involved in the accident, a copy of the accident report, any pictures of the vehicles involved, a list of doctors who treated the victim and the name of the treating hospitals, the name of any ambulance company that transported the victim to the hospital, the name of the health insurance company and contact address, the victim's employer and the amount of lost wages and/or benefits.

Have the victim's name, social security number, date of birth, letters of administration if any, health insurance, health history, funeral expenses (if applicable), witness names, dates of negligent treatment, all doctors, nurses and nursing assistants involved, and addresses of all brothers, sisters, children and other relatives of immediate family.
